We offer physical, occupational, and speech therapies so our residents can reach their highest level of functioning possible to return home or enjoy their favorite activities.
Physical Therapy
Physical therapy (PT) in a Skilled Nursing Facility (SNF) focuses on maintaining, promoting, or restoring lost physical function following a serious injury or illness. Physical therapists in a SNF typically work with physicians and other medical professionals to develop individualized therapy plans that address each resident’s unique medical needs.
PTs help residents with strength training to help improve mobility to resume normal activities such as lifting groceries and moving around home. Residents receiving PT also focuses on fall prevention by treating muscle weakness, and also address pain management with exercises that get to the root cause of the pain or discomfort.
Occupational Therapy
Occupational therapy (OT) uses assessments and intervention to help our residents learn to adapt, recover, or maintain meaningful activities after an injury or illness. Some activities OT may help residents with are grasping objects, prolonged standing, improving hand and eye coordination, and walking.
These practices help a resident with daily activities of living such as eating, bathing, dressing, doing laundry or other household chores. These are crucial skills for our residents returning home after therapy treatment.
Speech Therapy
Speech-Language Pathologists (SLPs) specializes in diagnosing and treating speech disorders such as aphasia (comprehension and communication disorder resulting from damage or injury to the specific area in the brain), dysphagia (condition with difficulty in swallowing food or liquid and may interfere in a person’s ability to eat and drink), and other communication disorders typically brought on by a stroke or other illness.
Our SLPs begin with an initial assessment and build a care plan based on each resident’s speech and language goals.
Short-Term Rehabilitation Discharge Planning
Our goal is to ensure any resident that is returning to their home, a loved one’s home or the community is adequately prepared. Our therapists endeavor to ensure each resident has the skills to return to their activities of daily living. Our social service teams can assist with making arrangements for assistive devices, community support and resources so each residents returning home is prepared to succeed and make a complete recovery.
Our clinical teams can assist with directions regarding wound care and medication management. Our goal is to see each resident being discharged to home is that they are functionally independent as possible and enjoying hobbies, friends, family, and their community.
